PROGRAM AGREEMENT
I understand that APMA Martial Arts is a martial arts training program that may offer school pickup as a convenience service. This is NOT a licensed childcare or after-school care program.
ASSUMPTION OF RISK
I understand that participation in martial arts training includes physical contact, strenuous activity, and risk of injury. I voluntarily assume all risks associated with participation.
RELEASE OF LIABILITY
I release and hold harmless APMA Martial Arts, its owners, instructors, staff, and affiliates from any injury, loss, accident, illness, or property damage that may occur during training, facility use, transportation, or events.
TRANSPORTATION WAIVER
I give permission for my child to be transported by APMA Martial Arts staff.
I understand that transportation is a courtesy service. APMA Martial Arts is not responsible for delays due to traffic, school dismissal changes, or unforeseen circumstances.
I release APMA Martial Arts from liability related to transportation.
ACADEMIC DISCLAIMER
APMA Martial Arts may provide time for quiet activities such as reading or school-related work.
This is NOT tutoring or academic instruction. No guarantee is made regarding completion or accuracy of schoolwork. Parents remain responsible for their child’s academic progress.
MEDICAL AUTHORIZATION
I authorize APMA Martial Arts staff to seek medical treatment and contact emergency services if necessary. I accept full responsibility for any medical expenses.
PHOTO / VIDEO RELEASE
I give permission for my child to appear in photos, videos, and promotional materials for APMA Martial Arts.
BEHAVIOR AGREEMENT
Students must follow rules and demonstrate respect at all times. Unsafe or disruptive behavior may result in suspension or removal without refund.
PAYMENT POLICY
Tuition is due in advance. Late payments may result in late fees. Missed days are non-refundable.

I have read and understand this agreement and voluntarily agree to all terms.

American Pride Martial Arts (APMA)

By enrolling in APMA, all students and parents/guardians acknowledge and agree to the following terms and conditions:


1. Respect & Behavior Standards

Students must demonstrate respect, discipline, and self-control at all times toward instructors, staff, and fellow students. Disruptive, aggressive, unsafe, or inappropriate behavior will not be tolerated.


2. Structured Discipline Policy

APMA maintains a structured discipline system which may include verbal warnings, time-outs, removal from activities, suspension, or permanent dismissal depending on the severity of the behavior.


3. Anti-Bullying & Zero Tolerance Policy

Bullying, harassment, intimidation, or physical aggression of any kind is strictly prohibited and may result in immediate removal from the program without prior warning.


4. Authority of Staff

All students and parents/guardians agree to respect and follow the direction of APMA instructors and staff at all times. Staff decisions regarding safety, discipline, and program structure are final.


5. Transportation Conduct & Safety 

Students participating in the free school pickup service must follow all safety rules while entering, exiting, and riding in vehicles.

Unsafe behavior including but not limited to:

  • Not remaining seated
  • Distracting the driver
  • Physical contact with other students
  • Ignoring staff instructions

may result in immediate suspension or permanent removal from transportation services.


6. Parent/Guardian Responsibilities

Parents/guardians agree to:

  • Maintain respectful communication with staff
  • Provide accurate and updated contact information
  • Ensure timely pickup of students
  • Support program rules and disciplinary decisions

7. Program Scope Disclaimer

APMA is a martial arts program. Any academic support or quiet time provided is a courtesy only and does not constitute tutoring, childcare services, or licensed after-school care.


8. Recording, Cameras & Facility Monitoring

For safety and security purposes, APMA facilities may be monitored by video surveillance. By participating in the program, you acknowledge and consent to this monitoring.


9. Zero Tolerance for Misconduct Allegations 

APMA maintains strict professional boundaries between staff and students.

Any serious allegations involving misconduct will be handled formally and may result in immediate suspension of the student’s participation during investigation.

False, misleading, or exaggerated claims that harm the reputation of the program, staff, or business may result in immediate dismissal.


10. Dismissal Policy 

APMA reserves the absolute right to suspend or dismiss any student or family at its sole discretion if behavior, conduct, or cooperation is deemed inappropriate, unsafe, or disruptive to the program environment.


11. Payment & Refund Policy Related to Dismissal

In the event of suspension or dismissal due to behavior or policy violations:

  • No refunds will be issued
  • Any remaining balances owed remain the responsibility of the parent/guardian

12. Agreement & Acknowledgment

By signing below, I confirm that I have read, understood, and agree to abide by all policies outlined in this Code of Conduct & Safety Agreement.

Failure to comply may result in disciplinary action, suspension, or removal from the program.
